Osteoarthritis is one of the most common joint diseases that is considered as a type of chronic non-infectious joint inflammation. Pain is the most important symptom of osteoarthritis, which is exacerbated by joint activity and weight bearing. Other symptoms complained of by patients include joint stiffness, a feeling of instability, limited mobility, and gradual disability. Considering the chronic nature of the disease and the need to use chemical drugs for a long time, these drugs cause significant side effects. The use of nonsteroidal anti-inflammatory drugs (NSAIDs) should be limited and used with caution due to internal side effects, especially gastrointestinal problems, adverse renal and anticoagulant effects. The use of topical analgesics in the form of ointments, creams and gels such as piroxicam, diclofenac and capsaicin, due to the chronic nature of the disease, is a more appropriate treatment and yields relatively acceptable results in controlling pain with minor side effects. Since the use of herbal medicines is more economical, more effective considering the presence of effective compounds, and also has fewer side effects than chemical drugs, they have been of interest for the treatment of this disease in recent years. In various studies, several medicinal plants such as bitter Almond oil, lilium candidum oil, commiphora myrrha, mentha longifolia, boswellia papyrifera, mumy, costus amarus, zataria multiflora, zingiber zerumbet, acorus calmus, laurus nobilis, and curcuma zedoaria have been introduced as analgesics. One of the constituents of the herbs of this drug is eugenol, a phenolic drug, the analgesic properties of which have been proved in various studies. Considering the structural similarity of eugenol with capsaicin and its agonist effect on GABA and antagonist effect on glutamate receptor, its administration seems to inhibit pain. Shefa dard Booalidaroo ointment (Jadvare morakkab) contains bitter almond oil, lilium candidum oil, commiphora myrrha, mentha longifolia,Boswellia papyrifera, mumy, costus amarus, zataria multiflora, zingiber zerumbet, acorus calmus, laurus nobilis, and curcuma zedoaria, which are used to treat joint pain. A double-blind clinical trial compared the effect of Shefa dard herbal ointment and diclofenac ointment on 93 patients with knee osteoarthritis. Patients were divided into two groups of diclofenac ointment and Shefa dard herbal ointment by block randomization. The treatment lasted for seven weeks and carried out three times a day by rubbing a little ointment on each knee. Pain level, limited mobility and stiffness of the knee joint, and daily function in patients were assessed by the Western Ontario and McMaster Universities Arthritis Index (WOMAC) at the beginning and end of the intervention. The results showed that the rate of reduction of joint pain and dryness and improvement of daily life function in the group of Shefa dard ointment was significantly different from diclofenac ointment group (P<0.05), so that the rate of improvement of symptoms, especially joint dryness in treatment with Shefa dard ointment was significantly greater than diclofenac ointment.
